Wellington Lipo 360 by Majestic Aesthetics is required by law to maintain the privacy of your protected health information, give you this notice, and follow the notice currently in effect.

Your rights

• Get a copy of your health and claims records — see medical records request
• Ask us to correct information you think is incorrect or incomplete
• Request confidential communications, such as a different phone number or address
• Ask us to limit what we use or share
• Get a list of those with whom we’ve shared information
• Get a copy of this notice on paper at any time
• Choose someone to act for you, such as a personal representative
• File a complaint if you believe your rights were violated

Your choices

You can tell us your preferences about sharing with family or close friends involved in your care. We will never use your information for marketing, sell it, or use your photos or story in promotion without your written authorization. Before/after photos for our gallery require a separate signed consent that you can revoke.

Sharing with professionals involved in your care, such as anesthesia providers and facility staff.

Quality improvement, scheduling, and training.

Billing

Processing payment for services, including financing partners you choose to use.

Other uses required or permitted by law

Our responsibilities

• We maintain the privacy and security of your protected health information
• We will let you know promptly if a breach occurs that may have compromised your information
• We will not use or share your information other than as described here unless you tell us we can in writing

Complaints

Contact our privacy officer at rn@majesticaesthetic.com, 561-363-5538. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.

Changes to this notice

We can change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available on request, in our office, and on this website.
